- How to access resources outside of typical referral patterns (e.g. tai chi, yoga, Alexander technique, strength training, cooking class, walking groups, etc.)?
- What are barriers to accessing these resources? Is a PCP referral necessary?
- What is the scope of home testing and monitoring? Ambulatory BP, Sleep, Glucose monitoring, Exercise? Diet?
